You're eligible for the PPO Part-Time and Retiree plan1 2. Or, you can enroll in the Regence full-time employee PPO plan and pay the difference in premium between the two plans. For a brief description of your benefits, check out your plan's 2009 Summary of Benefits (PDF).

1 2009 PPO Plan

If you live in the following counties, you will receive the Preferred Provider level of benefit only when you use a Preferred Provider:

Benton, Clackamas, Clatsop, Columbia, Coos, Douglas, Hood River, Lane, Lincoln, Linn, Marion, Multnomah, Polk, Wasco, Washington, Yamhill

Here’s an example of how benefits will be paid if you live in one of these counties and you use a Preferred or Participating Provider.

2 2009 PPO Plan in Rural Counties

If you live in the following designated rural counties, you will receive the Preferred Provider level of benefit when you see a Preferred or Participating Provider:

Baker, Crook, Curry, Deschutes, Gilliam, Grant, Harney, Jackson, Jefferson, Josephine, Klamath, Lake, Malheur, Morrow, Sherman, Tillamook, Umatilla, Union, Wallowa, and Wheeler

Please note: If you live in one of several zip code areas within certain counties just outside the Oregon border, you will receive the Preferred Provider level of benefit when you see a Preferred or Participating Provider. View the list of counties and zip code areas.
