Your Regence PEBB coverage pays for eligible nutritional counseling services as shown in the Summary of Benefits under physician visits.
The benefit is available as a total of four member visits per lifetime with no pre-qualifying conditions and four member visits per year with the following qualifying chronic conditions with a signed physician’s order form  is submitted:

• Heart Disease
• Cancer  
• Diabetes 
• Obesity
