Regence Disease Management is a health education and support program for people living with chronic health conditions. Offered at no extra cost to you, the Regence Disease Management program is designed to help you take a more active role in improving and maintaining your health by answering health care questions, helping you understand your doctor’s care and treatment plan, and providing educational materials and resources.
Examples of what to expect from the Regence Disease Management program:
- A member Welcome Packet that includes a letter to introduce and explain the program and a workbook or action plan
- Toll-free telephonic support from health care professionals such as registered nurses and dietitians
- Mailed reminders about important screening tests and standards of care
- Mailed personalized education such as disease-specific informational pieces, newsletters, and self-care goals
Members with a qualifying condition may be contacted by the program. The program is completely voluntary, and eligible health plan members who choose not to participate, can always join at a later date.
