Public Employees and Retirees | Chronic or Serious Conditions

Oregon Public Employees and Retirees

At Regence, we’re here to give you and your family the best possible health resources for leading a healthy life.  We have four great programs for managing a chronic condition, serious illness or injury.

Back on Track

Back on Track is a back health support program to help you and your adult dependents covered by Regence manage back pain. This program provides personalized back pain support and information to help you prevent back injury, manage back pain and make informed decisions about medical and surgical options. Rather than focusing on back surgery, Back on Track addresses the full range of low back pain care needs. Call your Regence Back Health Coach today for free, helpful information at 1 (888) 277-1544.

Nutritional Counseling

Learn how to eat better through covered sessions with a registered dietitian.  This benefit is available as a total of four member visits per lifetime with no pre-qualifying conditions and four member visits per year with certain qualifying chronic conditions.

Regence Education and Assessment for Diabetes Improvement

READI provides free monthly consultations with specially trained pharmacists who can help you stay on track with tests and medications, get needed screenings and meet goals related to nutrition, physical activity and other health concerns. You may also have access to generic versions of certain diabetes medications with no co-payment.

Regence Disease Management

Nurses and dietitians provide telephone and mail based support and education to help you understand and manage chronic health conditions such as diabetes, heart disease, asthma or arthritis.  Specially trained high risk care management nurses are also available if you require more complex management.

Case Management

We hope you’re never confronted with a serious medical problem, but if you are, a nurse case manager will provide the support and guidance you need. Case management can help in the event of a sudden or serious illness or injury. We’ll assign an experienced, compassionate nurse case manager to serve as your personal advocate during a time when you need it most, help you understand your treatment options, show you how to get the most out of your benefits, and work with your physician to support your treatment plan. For more information or to speak with a case manager, call 1 (866) 543-5765.
