Preferred Choice Sixty-Five is available in Curry, Douglas and Tillamook counties.

Additional Information

In- and Out-of-network Coverage
Services other than emergency and urgently-needed services must be obtained within the Preferred Choice Sixty-Five network. Members will be responsible for payment of all Medicare deductibles and coinsurance, as well as any additional charges as prescribed by the Medicare program, if services as outlined above are not received within the Preferred Choice Sixty-Five network. Members will also be liable for any charges not covered by the Medicare program.

 

Annual Renewal of Contract
Regence BlueCross BlueShield of Oregon contracts annually with CMS to provide Preferred Choice Sixty-Five.

 

Individual Eligibility
Anyone residing in the service area and who is eligible for Medicare benefits under Part A or is enrolled in Part B or Part B only is eligible to enroll.
