Why You Should Use a Preferred Network Provider
- Preferred network providers file your claims for you.
- You pay less for health care if you use Preferred network providers. Your benefits are enhanced so that when you see a Preferred provider for an office visits, you pay a fixed-dollar copayment for your share of the cost, with no deductible, in most situations.
- If you use a non-participating professional provider, your portion of the charge after our payment could be significant after a deductible.
BASIC OPTION
Why You Should Use a Preferred Network Provider
- Preferred network providers are the only approved and covered providers for Basic Option benefits.
- If you choose to use an out of network provider, benefits are not payable except in certain situations, such as emergency care.
- Preferred network providers file your claims for you.
IMPORTANT MESSAGE
It is important to know that when you enroll in this Plan, benefits are available as described in the brochure, but the continued participation of any one doctor, hospital, or other provider cannot be guaranteed.
Please contact the Customer Service number listed on the back of your member card to check on the Preferred status of any FEP provider.
