  • Your personal situation has changed (such as income or family status)
  • You want to look for a different plan
  • Your plan has changed or is no longer offered

Next, go to Shop and select a plan, but don't go through the checkout process.

Then, complete and send your form in by mail, FAX or email. It must be in our office on the date below.

Yes, I bought my Regence plan on the exchange

Go to Shop to review 2017 plans and pick a new one. (If your income or family has changed and you don't see the option to shop exchange plans on our site, contact the exchange.)
