Medication Therapy Management program

Taking multiple medications can increase the risk of drug interactions, side effects and other complications. Through the Medication Therapy Management (MTM) program, you can get one-on-one guidance from a pharmacist or other trained health care provider. They’ll make sure your medications are safe and working for you—and may help you find lower-cost options.

What's included

You can receive the following services through the MTM program, which is provided at no additional cost to Medicare members and is not considered a benefit.

You’ll receive an action plan and personal medication list (PDF) to help you keep track of your medications. Make sure to share this information with your care team at your next annual wellness visit.

Your medications will be reviewed every three months, and we'll reach out if we identify any new problems.

Who qualifies

To qualify for the MTM program, you must be a Medicare member who meets all three of the following requirements:

You have a total drug cost of at least $5,330 a year (paid by you and your plan).

You take eight or more routine medications covered by your Medicare Part D plan.

You have at least three of the following chronic conditions:

Heart failure, diabetes, high cholesterol, high blood pressure, osteoporosis, rheumatoid arthritis, asthma, COPD (chronic obstructive pulmonary disease), HIV/AIDS, depression, bipolar disorder, schizophrenia.

You may also qualify for the MTM program if you’re currently enrolled in a Drug Management Program to better manage and safely use select medications such as opioids.

How do I know if I qualify?

What happens during the comprehensive medication review?

The pharmacist or trained health care provider will:

  • Go over your medications with you
  • Make sure your medications are safe for you
  • Answer questions or discuss concerns you may have about your medications
  • Discuss lower-cost medications if they're available
  • Help you find ways to stay on track with your medication therapy

When should I complete the comprehensive medication review?

Complete your comprehensive medication review prior to your annual wellness visit for the most benefit. However, if you already have had your annual wellness visit, it’s still important to complete the review this year. We’ll communicate with your doctor as needed to ensure your medications are working well for you.

Will my doctor be in the loop if any changes need to be made to my prescriptions?

Yes. Your MTM pharmacist or trained health care provider may make recommendations to you and your doctors—but only your doctors can change your prescription.

Am I required to participate in the MTM program?

No. The MTM program is an added service. We're here to help, but you're not required to participate. You can opt out of the MTM program at any time by calling 1 (800) 541-8981. TTY users should call 711. Our hours are 8 a.m. to 8 p.m. Monday through Friday. From Oct. 1 through March 31, we are available 8 a.m. to 8 p.m. seven days a week.
