Medication Therapy Management program

Taking multiple medications can increase the risk of drug interactions, side effects and other complications. Through the Medication Therapy Management (MTM) program, you can get one-on-one guidance from a pharmacist or other trained health care provider. They’ll make sure your medications are safe and working for you—and may help you find lower-cost options.

What's included

You can receive the following services through the MTM program, which is provided at no additional cost to Medicare members and is not considered a benefit.

Pill bottle

An evaluation

You’ll have a comprehensive medication review with a pharmacist or other trained health care provider (15-30 minutes).

Paperwork

A plan and a list

You’ll receive an action plan and personal medication list (PDF) to help you keep track of your medications and share information with your care team.

Your medications will be reviewed every three months, and we'll reach out if we identify any new problems.
