Palliative Care

The Palliative Care program provides easy access to one-on-one support for members who face a serious or life-limiting condition. The program provides educational materials, support, guidance and a personal advocate to help you and your family navigate the health care system when you need it most.

Services include:

• Clinical social worker visits
• Home health visits to provide care such as therapies and nursing in the home, as well as personal care
• Medical team conferences

• Care coordination such as hospital, hospice facility and other programs in your community

This program is offered at no additional cost to our Medicare Advantage HMO and PPO members. These services are also accessible whether or not an individual is seeking hospice care.
