Enroll by U.S. mail or fax

Choose your plan from the forms and documents page. Download, print and fill out a separate application for each person you wish to enroll. Mail to the address listed on the form or fax to 1 (888) 335-2988.

You can also enroll in Regence plans by visiting www.medicare.gov

Know when to enroll

Your Initial Enrollment Period is the time period you have to sign up for Medicare when you first become eligible, through either age or disability. You have a seven-month window to enroll in Medicare: the month you become eligible, the three months prior, and the three months after.
