DispatchHealth launching Bridge Care service to care for members after discharge

On-call medical care delivered to members’ homes

When dealing with a serious injury or illness, many Regence members have access to on-call, at-home care for high-acuity medical needs through DispatchHealth. When a member calls DispatchHealth, a team of trained emergency medical professionals will visit them at home and provide treatment for a variety of conditions, including:

  • Cuts, lacerations and abrasions
  • Infections
  • Mild to moderate abdominal pains
  • Severe cold and flu symptoms
  • Sprains and strains.

DispatchHealth professionals can call in prescriptions at a member’s local pharmacy, update the member’s doctor and submit the claim to us. It’s an easy, reliable and convenient way for members to get in-network, in-home care, and it’s available throughout the Portland, Seattle-Tacoma and Spokane metropolitan areas.

Introducing Bridge Care: In-home care for members after discharge

The days immediately following a stay in a hospital, skilled nursing facility (SNF) or inpatient rehabilitation facility, whether planned or unplanned, can be a critical time in a member's recovery. It can also be a confusing, vulnerable time.

Beginning July 15, DispatchHealth will provide Bridge Care for members within 72 hours of discharge. Bridge Care is for moderate to high-risk patients to help bridge them back to their primary care provider (PCP) or specialist. In a Bridge Care visit, DispatchHealth’s medical team will visit the member at home, assess their status and make sure they have everything they need to recover. The goal is to help prevent readmission.

If a member has ongoing symptoms that need management, the team can intervene and provide needed medical care on-site.

Bridge Care visits can be arranged by the patient’s hospital care team, PCP or SNF and scheduled with the patient directly by DispatchHealth. In most cases, a Bridge Care visit will be billed through the member's provider; otherwise, the visit is billed directly through Regence.

A Bridge Care visit isn't intended to replace care from a primary care provider (PCP) or specialist. It's truly an effective, convenient bridge to help get members from discharge to their PCP and back out into their community.
