Heartbeat Health

Heartbeat Health is a national cardiology practice that helps prevent, manage and treat heart conditions from anywhere. From checkups to follow-ups, they make heart care easy and convenient for patients.

It is available to commercial and Medicare Advantage members.

What is Heartbeat Health?
Heartbeat Health partners directly with providers and health plans to identify members at higher risk of cardiac events or who have recently experienced a cardiac event. Then, its staff of experienced cardiologists, nurse practitioners, physician assistants and registered nurses provide services such as remote diagnostics, virtual consultations, medication management, post-discharge follow-up and more.

What services do they offer?
They provide comprehensive heart care, starting with virtual visits to address a patient’s needs. Their team coordinates necessary testing, manages medications and works closely with the patient’s providers for advanced care when necessary.

They offer:

  • Early disease detection: Introduction of diagnostic programs with wrap-around care to support new diagnoses.
  • Longitudinal care programs: Virtual first care, including medication management, remote monitoring and care coordination for conditions such as atrial fibrillation, heart failure and vascular disease.
  • Post-discharge care: Development and support of a comprehensive care plan aimed at reducing readmission for post-acute cardiac patients.

Who should be referred to Heartbeat Health?
Heartbeat Health can help providers with:

  • Medical management for adults (18+) with comorbidities like hypertension, type 2 diabetes, heart failure, coronary disease, arrhythmias or high cholesterol
  • Patients that need a workup for symptoms such as chest pain, dyspnea, palpitations or syncope
  • Pre-operative assessments or post-discharge follow-up
  • Individuals facing long wait times or limited access to local cardiology services
