Quality Improvement Toolkit

Surveys such as the Consumer Assessment of Healthcare Providers and Systems (CAHPS®)/Health Outcomes Survey (HOS) are used to gauge the members’ experience with their provider and focus on certain health topics.

CAHPS/HOS scores account for more than a quarter of a health plan’s overall Star Ratings from the Centers for Medicare & Medicaid Services (CMS). When these ratings improve, it’s an indicator that Regence patients are having meaningful conversations with their providers and receiving helpful information during their provider visits to lead healthier lives.

This toolkit was designed to help you have memorable conversations with your patients. The resources focus on various health topics for delivering the best possible care.

  • Maintaining a healthy weight
  • Medications and member experience with medications
  • Monitoring physical activity
  • Overall rating of health care

  • Wellness and preventive visits for Medicare Advantage members

  • Help you understand how the care your patients receive impacts these scores
  • Provide information about how access to care and care coordination can improve patient outcomes

Using the toolkit

Filter options for Categories/Year/Languages:

  • Select the drop-down Categories menu to see each topic, including best practices, member benefits and other resources.
  • Check a box (or multiple boxes) for topic(s)
    o Examples include: Advanced care planning, bronchitis or low back pain
  • Click Year (select “All years”)
  • Optional: Click Languages to filter if desired

Search bar:

  • Use the search option if you want to go directly to a flyer (not all flyers have the topic in the title and using the search bar bypasses the category information)

Important notes:

  • Selecting the “All Documents” category:
    • Provides an alphabetic list of flyers by title and language
    • Does not show additional information about the topic, like best practices, member benefits and other resources
    • Does not give the year
    • Does not show which category a flyer is associated with
