How does an AHN work?

For a person with type 2 diabetes who needs help from multiple providers, it's important that all agree on the care plan. The patient would choose a PCP to be point person—to keep the other providers informed about tests, prescriptions, medical supplies—and coordinate with specialists on managing care.

This means better communication, which could lead to great care at a lower price.

Choosing a PCP

A PCP gets to know you to help you make the best decisions about your health. You can choose a PCP based on specialty or even location, so that getting care is easier and more convenient for you.

Here are examples of providers you can choose as your PCP:

  • Family practice
  • General practice
  • Internal medicine
  • Pediatrician
  • OBGYN

How to find a PCP

Find a PCP in just three steps:

  1. Sign in on regence.com (registration required).
  2. Go to Find a doctor.
  3. Search for a doctor by name or specialty.

Each member of your family can choose a different PCP, and you can change as often as you'd like.

Tell Regence your PCP choice

We include your PCP's name on your member ID card, so it's important for you to let us know who will be managing your care. If you change your PCP, you'll receive a new member ID card that shows the new name. Let us know who your PCP is in one of the following ways:

  • Fill out the online form.
  • Call us at 1 (888) 675-6570 (TTY: 711).
  • Email us at cs@regence.com.
  • Go to Message Center, compose a new message and select Change my primary care provider as the topic.
